The ketogenic diet, a high-fat, moderate-protein, low-carbohydrate diet, can induce weight loss and improve glycemic control but also poses risks of hyperlipidemia, elevation of liver enzymes, and the onset of fatty liver disease. A keto diet may lead to elevated AST and ALT levels, indicating liver damage or inflammation. Rapid weight loss, high fat intake, and nutrient deficiencies can contribute to increased AST. A study revealed that feeding a high-fat ketogenic diet increased cholesterol accumulation in the liver, which further enhanced the carbon tetrachloride (CCl 4)- and bilirubin (BTH) levels.

A keto diet does not cause non-alcoholic fatty liver disease (NAFLD), and people with NAFLD may see significant improvements. Resolution of liver enzymes was seen within eight months and LDL-C levels normalized one year later. However, caution should be exercised as a ketogenic diet can elevate liver enzymes and worsen lipid profiles in patients with NAFLD.

The ketogenic diet has been shown to reduce insulin levels and lipogenesis, but it can also increase cholesterol quantity in the liver, thereby enhancing liver inflammation and reduced antioxidant levels. Animal studies suggest that the keto diet can cause NAFLD and raise liver enzyme levels.

In addition, there is limited data on short-term impact on lipids and liver enzymes in patients who start ketogenic diets. This case report highlights an uncommon case of ketogenic diet-induced elevated cholesterol, elevated liver enzymes, and potential non-alcoholic fatty liver disease.

The study highlights the importance of periodic fasting in managing chronic liver disorders, including non-alcoholic fatty liver disease (NAFLD), obesity, overweight, and fatty liver index.
