Treatments for these conditions vary, and an accurate diagnosis requires testing for the release of these enzymes in the blood. A biomarker test for troponin is the primary test healthcare providers use to detect heart damage from a heart attack or ACS. Troponin levels can rise for up to 12 hours after a heart attack. They stay elevated for up to two weeks. You may get several cardiac enzyme tests spaced several hours or days apart to measure these biomarkers.

Healthcare providers may also test for natriuretic peptides (NPs) that may be elevated in conditions like heart failure. Providers can measure various types of peptides, but they most commonly use N-terminal proBNP (NT-proBNP) in the assessment of strain on the heart due to heart failure.

A cath lab is staffed by a team of different specialists, usually led by a cardiologist. What is a creatine kinase (CK) test?. This test measures the amount of creatine kinase (CK) in a sample of your blood. CK is also called creatine phosphokinase (CPK).

CK is an enzyme. An enzyme is a protein that speeds up certain chemical reactions in your body. CK helps make energy. Most of the CK in your body is in your skeletal muscles. These are the muscles you use to move. You also have CK in your heart muscle and small amounts in your brain.

Normally, a small amount of CK gets into your blood from the usual wear and tear on your muscles. But if your muscles, heart, or brain tissues are damaged, larger amounts of CK leak out of your cells into your bloodstream.

A CK test is mainly used to help diagnose and monitor injuries and diseases that damage skeletal muscles and cause high levels of CK in your blood. But it may also be used for conditions that damage the heart muscle and the brain.

Cardiac troponin. This protein is by far the most commonly used biomarker. It has the highest known sensitivity. It enters into your bloodstream soon after a heart attack. It also stays in your bloodstream days after all other biomarkers go back to normal levels. Two forms of troponin may be measured: troponin T and troponin I. Troponin I is highly specific to the heart and stays higher longer than creatinine kinase-MB. Current guidelines from the American Heart Association (AHA) say this is the best biomarker for finding a heart attack. The AHA says to limit use of the other biomarkers. These include CK, CK-MB, and myoglobin.

Creatinine kinase (CK). This enzyme can also be measured several times over a 24-hour period. It will often at least double if you’ve had a heart attack. But because levels of CK can go up in many other conditions besides a heart attack, it is not very specific.

CK-MB. This is a subtype of CK. It is more sensitive for finding heart damage from a heart attack. CK-MB rises 4 to 6 hours after a heart attack. But it is generally back to normal in a day or two. Because of this, it’s not helpful when a healthcare provider is trying to figure out if your recent chest pain was a heart attack.
