These are chemical messengers that travel through your blood. Pancreatic hormones help regulate your blood sugar levels and appetite, stimulate stomach acids, and tell your stomach when to empty.

Pancreatic enzymes. Your pancreas creates natural juices called pancreatic enzymes to break down foods. These juices travel through your pancreas via ducts. They empty into the upper part of your small intestine called the duodenum. Each day, your pancreas makes about 8 ounces of digestive juice filled with enzymes. These are the different enzymes:

Lipase. This enzyme works together with bile, which your liver produces, to break down fat in your diet. If you don’t have enough lipase, your body will have trouble absorbing fat and the important fat-soluble vitamins (A, D, E, K). The human pancreas has the largest capacity for protein synthesis, with much of this being devoted to the synthesis of digestive enzymes secreted in the intestinal lumen. These enzymes, including proteolytic, amylolytic, lipolytic, and nuclease, can potentially digest cells and cause significant damage. However, there are mechanisms to prevent these enzymes from digesting the pancreas, such as storage and packing in acidic zymogen granules to inhibit activity and synthesis and storage as inactive precursor forms.

Some enzymes are stored in the pancreas as inactive proenzymes before secretion, which are activated when they enter the duodenum. Enterokinase, a brush-border glycoprotein peptidase, activates trypsinogen by removing an N-terminal hexapeptide fragment of the molecule. The active form, trypsin, catalyzes the activation of other inactive proenzymes.

Key digestive enzymes, such as α-amylase and lipase, are present in the pancreas in their active forms, which may not cause damage if released into pancreatic tissue. Inactive proenzymes called zymogens enter the duodenum, where enterokinase leaves trypsinogen, activating it to trypsin. Trypsin converts zymogens into other enzymes.

Two of the main pancreatic hormones are insulin, which acts to lower blood sugar, and glucagon, which acts to raise blood sugar. Maintaining proper blood sugar levels is crucial to the functioning of key organs including the brain, liver, and kidneys.

Diseases of the Pancreas. Disorders affecting the pancreas include pancreatitis, precancerous conditions such as PanIN and IPMN, and pancreatic cancer. Each disorder may exhibit different symptoms and requires different treatments.

These enzymes function optimally in the more basic environment of the small intestine, where the pH ranges from 6 to 7 due to bicarbonate secreted by the pancreas.

Peptidases are precursors to active peptidases, which are converted to trypsin, chymotrypsin, carboxypeptidase, and elastase, respectively. Enterokinase, a duodenal enzyme, converts trypsinogen to trypsin, which can then convert chymotrypsinogen, procarboxypeptidase, and proelastase to their active forms.

These pancreatic zymogens leave the pancreas through the main pancreatic duct (of Wirsung) and join the common bile duct, forming the ampulla of Vater and emptying into the descending portion of the duodenum via the major duodenal papilla. Bile contains a mixture of bile salts, cholesterol, fatty acids, bilirubin, and electrolytes that help emulsify hydrophobic lipids in the small intestine.

Once in the duodenum, an activation cascade begins with enterokinase produced by the duodenum to activate trypsinogen to trypsin, and trypsin activates the other pancreatic peptidases. The duodenum also contributes digestive enzymes, such as disaccharidases and dipeptidase.

The gastrointestinal system includes the mouth, stomach, and small intestine, which break down fat, carbohydrates, and large proteins into absorbable forms for bodily use.
