The activity of an enzyme is sensitive to temperature and pH, with each enzyme having an optimal pH. A change in pH can alter the ionization of the R groups of amino acids, leading to changes in the structure of the enzyme. Enteric-coated pancreatic enzymes are most effective at a pH > 6, but in patients with CF, the duodenal pH is < 6. Acid-suppression medications can increase gastric pH.
Enzymes have a more or less narrow optimal pH depending on their environment. Pepsin is active in the stomach, which is acidic, with an optimal pH of 2.0. However, pancreatic lipase activity and other hydrolytic enzymes in the small intestine favor a more neutral pH, so processes must be in place to quickly and effectively raise intestinal pH.
Purified human pancreatic alpha-amylase (alpha-1,4-glucan 4-glucano-hydrolase) is stable over a wide range of pH values (5.0 to 10.5) with an optimal pH for the stomach. Enzymes like amylase, lipase, and proteases work best at a pH of around 7.0 to 8.0. The pancreas releases these enzymes along with a bicarbonate-rich juice that neutralizes the acidic chyme from the stomach.
Enzymes work inside and outside cells, with cell pH maintained at 7.0 to 7.4. Cellular enzymes will work best within this pH range, followed by disintegration of the enteric coating and enzyme release at pH 5.5, mimicking duodenal conditions. Different parts of the digestive system produce different enzymes, with an ideal pancreatic enzyme preparation disintegrating immediately once it reaches the duodenum (pH 6–7).

Above pH 7, the activity of amylase rapidly decreases beacuse the concentration of H+ ions (or protons) is too low. This change in (H+) alters the interactions between the variable R-groups of the amino acids in the protein chains that make up the enzyme. These interactions include hydrogen bonds, ionic bonds and Van der Waals forces. Their function is to maintain and stabilise the tertiary 3D structure of the proteins of the enzyme, therefore giving the enzyme its specific shape, especially in its active site where its substrate must bind for the reaction to be catalysed. If these stabilising bonds are altered by a change in pH, the shape of the enzyme’s active site will also change. This means that the sunstrate is no longer able to bind to the active site and the reaction will not take place, therefore decreasing the activity of amylase.
