Thickening is a common practice in NICUs and other clinical settings to manage dysphagia and GERD, but its use can have numerous consequences, including non-equilibrium, hypernatremia, nutrient malabsorption, constipation, dehydration, delayed gastrointestinal transit, fatigue from nipple extraction, difficulty extracting liquid, decreased oral intake, and prolonged transition of oral feeding in the presence of oropharyngeal dysphagia. Additionally, each 1 tsp/oz of rice cereal or oatmeal adds 5 kcal/oz, which can lead to excessive body fat deposits. Thickeners are often used to treat dysphagia, as they decrease flow rate and increase oropharyngeal transit time to improve airway protection and oromotor control. However, milk extraction can be problematic due to non-homogeneous mixtures or nipple hole diameter, which may contribute to dysphagia. Thickening liquids may also increase post-swallow pharyngeal residue, contributing to delayed pharyngo-esophageal transit. Additionally, thickened feeds may aspirate, which can have significant pulmonary consequences. The inherent reason for dysphagia cannot be fully addressed by adding thickener, and the risks may outweigh any potential benefits. Despite limited evidence, thickening remains a common practice in NICUs and other clinical settings, highlighting the need for developing innovative treatment strategies for complex NICU infants with dysphagia and/or GERD.

  • Are they bad? As you’ll learn below, some of these additives are worse than others, but all of them are unnecessary. The worst ones you wouldn’t want to touch – let alone drink.
  • Are they safe? The FDA approves many thickeners and emulsifiers. So, technically yes. But there are enough concerns that we like to stay away from them completely.
  • Are they natural? Yes and no. While they come from plants, many gums are ultra-processed.
  • How do they affect my body? Studies have shown interesting effects on animals and humans, ranging from digestive discomfort and bloating to increased risk of infection.

Generally, gums, emulsifiers and thickeners are a sign that plant-based foods are low quality. There is a big difference between an almond milk with 1g of protein and 12 hard-to-pronounce ingredients (some of which prevent nutrient absorption) vs. an almond milk with 5g of protein and just two simple ingredients.

Pay special attention to the arduous task necessary to create these compounds; and the level of scientific testing needed to merely deem whether they are actually fit for human consumption.

  • Slowly add level measured thickener to liquid, stirring with fork or whisk as you pour.
  • Stir briskly until thickener has dissolved.

While there are clear benefits of thickened liquids for someone with dysphagia, there are also several disadvantages that should be considered. This article will explore the evidence surrounding the pros and cons of thickened fluids and the implications it has for those living with dysphagia. Thicker is not always safer.

Thickened fluids are fluids that are of a thicker consistency than thin fluids. Thin fluids are fluids with the same consistency as water. There are thickening products available, such as powders and liquids, that can thicken thin fluids when added to them. These products use starches and gums as the thickener, and typically have little to no taste. Additionally, there are fluids that are naturally thick: such as fruit nectar juices and milkshakes.

Thickened fluids are often recommended for those who have dysphagia. Drinking thickened fluids can assist in preventing fluid entering the airway and lungs, also known as aspiration. Thickened fluids move slower than thin fluids, therefore allowing your body more time to move the muscles in your throat and close and protect your airway as the fluid moves down into your oesophagus. It also allows greater control of the fluid in the mouth, to avoid it pre-maturely going into your airway or oesophagus. Thin fluids require more coordination and strength to swallow than thickened fluids. I’m sure we’ve all had our water go down the wrong pipe before.

A person receives essentially the same hydration as if they had consumed a same amount of non-thickened fluid. We recommend you consume as much liquid as you would with non-thickened fluid. It is important for a person to receive a significant liquid intake on a daily basis, unless a physician has advised to limit fluids.

Abstract. Background: Thickened milk formulas are used to treat infants with gastroesophageal reflux (GER), but these substances often increase the duration of reflux episodes and worsen symptoms, and they have been associated with diarrhea, constipation, and cough.

Objectives: The aims of this study were to determine the efficacy of an antiregurgitation milk formula in the clinical and laboratory setting in infants with proved GER, to investigate any possible adverse events (cough and change in the number of bowel movements or the consistency of stools), and to identify its effects on height and body weight.

Methods: Infants with recurrent vomiting and GER who were not responsive to standard treatment were eligible for the study. Infants in the treatment group (group A) were managed for 4 weeks with a specific antiregurgitation milk formula (with cornstarch and an increased amount of casein), and those in the control group (group B) were given a standard milk formula. The number of episodes of vomiting, regurgitation, and cough, as well as the frequency and consistency of stool, height, and body weight were noted at least 10 days before and during the study. A second pH monitoring was performed after 4 weeks in both groups.

These include: increased risk of dehydration and urinary tract infections, reduced health and social-related quality of life, early satiety, unpalatable taste and texture, increased thirst, altered bowel habits, increased risk of oral and pharyngeal residue, impaired bioavailability of certain medications, and increased risk of respiratory tract infections if thickened fluids are aspirated.

Patient-reported outcome studies generally indicate a preference against thickened fluids and highlight difficulties with adherence to thickened fluid recommendations.

Clinical practitioners have a responsibility to familiarise themselves with the current literature and explore within their workplaces how their use of thickened fluids aligns with the evidence base.
