Pancreatitis is an inflammation of the pancreas, a long, flat gland behind the stomach that helps the body digest food and regulate blood sugars. It can cause swelling, pain, and changes in organ or tissue function. Enzymes and insulin may need to be replaced to maintain the body’s function. Chronic pancreatitis requires enzyme supplements and insulin injections to replace the enzymes and insulin produced by the pancreas. Enzymes work for about 45 to 60 minutes after taking them, and complications of exocrine pancreatic insufficiency (EPI) can lead to a life expectancy of 8 years less than the general population. About 25 patients with the Whipple procedure will have long-term malabsorption and require supplemental enzymes for life. Pancreatic enzyme replacement therapy is safe and effective in treating pancreatic exocrine insufficiency.

In acute pancreatitis, amylase can rise rapidly within three to six hours of symptoms onset and may remain elevated for up to five days. Lipase, however, can cause serious complications and even death. It is possible to treat exocrine pancreatic insufficiency (EPI) successfully, but without treatment, it can cause serious complications and even death.

Abstract. Benign pancreatic hyperenzymemia (BPH) or Gullo’s syndrome is a benign condition consisting of an oscillating elevation of pancreatic enzymes without the identification of pancreatic disease. Its diagnosis is usually incidental and by excluding other conditions that occur with elevated pancreatic enzymes. To the best of our knowledge, there are no reports of this diagnosis to this day in Portugal.

A 65-year-old female was referred to an internal medicine consultation for complaints of xerostomia, xerophthalmia, and xeroderma with one year of evolution. From the study carried out by the patient prior to the consultation, an incidental elevation of amylase stands out. The sicca symptoms were attributed to sertraline since, after excluding other causes, its discontinuation resolved the symptoms. Regarding the elevation of pancreatic enzymes, the patient underwent an extensive diagnostic study for clarification without identifying any condition. The serial measurement of amylase and lipase revealed an oscillating increase in pancreatic enzymes with temporary normalization. After one year of follow-up, the diagnosis of Gullo’s syndrome was established.

The identification of Gullo’s syndrome is extremely important as it avoids carrying out unnecessary tests in the future and allows the patient to be reassured in the face of this benign alteration of pancreatic enzymes. A follow-up of at least one year is crucial since some pancreatic tumors course with an asymptomatic increase in pancreatic enzymes.

Rationale and Comments. Amylase and lipase are digestive enzymes normally released from the acinar cells of the exocrine pancreas into the duodenum. Following injury to the pancreas, these enzymes are released into the circulation. While amylase is cleared in the urine, lipase is reabsorbed back into the circulation. In cases of acute pancreatitis, serum activity for both enzymes is greatly increased. Serum lipase is now the preferred test due to its improved sensitivity, particularly in alcohol-induced pancreatitis. Its prolonged elevation creates a wider diagnostic window than amylase. In acute pancreatitis, amylase can rise rapidly within three to six hours of the onset of symptoms and may remain elevated for up to five days. Lipase, however, usually peaks at 24 hours with serum concentrations remaining elevated for eight to 14 days. This means it is far more useful than amylase when the clinical presentation or testing has been delayed for more than 24 hours. Current guidelines and recommendations indicate that lipase should be preferred over total and pancreatic amylase for the initial diagnosis of acute pancreatitis and that the assessment should not be repeated over time to monitor disease prognosis. Repeat testing should be considered only when the patient has signs and symptoms of persisting pancreatic or peripancreatic inflammation, blockage of the pancreatic duct, or development of a pseudocyst. Testing both amylase and lipase is generally discouraged because it increases costs while only marginally improving diagnostic efficiency compared to either marker alone.

What Are Enzymes and How Do They Work?. Most pancreatic enzyme supplements come in capsule form. Inside each capsule are many small beads that contain the digestive enzymes. Each bead is covered with a special enteric coating. This coating allows the beads to dissolve in the small intestine. The digestive enzymes are then released in the small intestine to help digest food. Enzymes work for about 45 to 60 minutes after taking them.
