Digestive enzymes are proteins in the body that help break down food. They are released when we anticipate eating, smell and taste food, and go through the digestive process. The gastric glands contain different types of cells that secrete various enzymes, including hydrochloride acid, which activates the protein-digesting enzyme pepsin. Gastric secretion occurs in three phases: cephalic, gastric, and intestinal. During each phase, the secretion of gastric juice can be stimulated or inhibited.

The daily intake of protein in the western diet averages 100 g per day, with an additional 35 g contributed by salivary and GI secretions. Low pH in the stomach activates pepsinogen to break down proteins. The action of the enzymes is promoted by hydrochloric acid (HCl) secreted by the stomach and bile from the liver. The mucosal cells in the small intestines are called enterocytes. The hydrogen (H) and chloride (Cl) components of HCl are secreted separately by hydrogen/potassium ATPase pumps and chloride channels in the stomach.

Gastric lipase is an acidic lipase secreted by the gastric chief cells in the fundic mucosa in the stomach, with a pH optimum of 3–6.   • Pepsin : Pepsin is secreted by the stomach to break down proteins into peptides, or smaller groupings of amino acids. Those amino acids are then either absorbed or broken down further in the small intestine.
  • Trypsin : Trypsin forms when an enzyme secreted by the pancreas is activated by an enzyme in the small intestine. Trypsin then activates additional pancreatic enzymes, such as carboxypeptidase and chymotrypsin, to help break down peptides.
  • Chymotrypsin : This enzyme breaks down peptides into free amino acids that can be absorbed by the intestinal wall.

Sucrase. Sucrase is secreted by the small intestine, where it breaks down sucrose (the sugar in table sugar) into fructose and glucose. These are simpler sugars that the body can absorb.

The secretion of acid is under the regulation of both hormonal and neural components, including gastrin, histamine, prostaglandins, somatostatin, gastric inhibitory polypeptide, secretin, and the vagus nerve. Interventional inhibition of acid secretion to avoid various complications of excess acid is commonly done by administering proton pump inhibitors.

The acidic environment of the stomach is not only useful for protein denaturing but also for protection against potentially infectious agents. All material consumed by the body must pass through the stomach, making it an important defense against microbes. Many bacteria are killed or inhibited by the stomach’s acidity.

Additionally, secretory cells of the gastric glands include foveolar cells and enteroendocrine cells. Foveolar cells protect the stomach from the corrosive nature of its acidic environment by producing mucus and bicarbonate (HCO3). Enteroendocrine cells secrete various digestive hormones such as gastrin, somatostatin, and ghrelin. Gastrin release occurs in response to increased gastric distension, increased gastric pH, and the presence of amino acids in the stomach.

The process of gastric secretion can be divided into three phases (cephalic, gastric, and intestinal) that depend upon the primary mechanisms that cause the gastric mucosa to secrete gastric juice. The phases of gastric secretion overlap, and there is an interrelation and some interdependence between the neural and humoral pathways.

The cephalic phase of gastric secretion occurs in response to stimuli received by the senses—that is, taste, smell, sight, and sound. This phase of gastric secretion is entirely reflex in origin and is mediated by the vagus (10th cranial) nerve. Gastric juice is secreted in response to vagal stimulation, either directly by electrical impulses or indirectly by stimuli received through the senses. Ivan Petrovich Pavlov, the Russian physiologist, originally demonstrated this method of gastric secretion in a now-famous experiment with dogs.

The gastric phase is mediated by the vagus nerve and by the release of gastrin. The acidity of the gastric contents after a meal is buffered by proteins so that overall it remains around pH3 (acidic) for approximately 90 minutes. Acid continues to be secreted during the gastric phase in response to distension and to the peptides and amino acids that are liberated from protein as digestion proceeds. The chemical action of free amino acids and peptides excites the liberation of gastrin from the antrum into the circulation. Thus, there are mechanical, chemical, and hormonal factors contributing to the gastric secretory response to eating. This phase continues until the food has left the stomach.

  • Pepsin is the main gastric enzyme. It is produced by the stomach cells called “chief cells” in its inactive form pepsinogen, which is a zymogen. Pepsinogen is then activated by the stomach acid into its active form, pepsin. Pepsin breaks down the protein in the food into smaller particles, such as peptide fragments and amino acids. Protein digestion, therefore, primarily starts in the stomach, unlike carbohydrate and lipids, which start their digestion in the mouth (however, trace amounts of the enzyme kallikrein, which catabolises certain protein, is found in saliva in the mouth).
